Selecting the Select Your Plan "O" at the bottom of any plan column, and then the Enroll Now button will launch the Application Form. Complete all required information including payment options. At the end you will have an opportunity to download "plan documents."

Spirit Dental
Spirit Dental uses the Ameritas Classic Dental Network. Plans differ state to state. I have included links below to their individual quote page and network lookup tool. While their network is smaller than above; they offer three cleanings annually with two exams AND coverage starts tomorrow!
Check to see if your dentist is in their network before starting a quote. Enter your Zip Code and "individual, +1, or family" to bring up details of plans available in your area. Also, when the quote starts, you will be given the option to add vision insurance before starting the application.
Ameritas
Enjoy next-day coverage on most dental insurance plans. There are no enrollment fees with any plan. Preventive visits, such as exams and cleanings, are covered up to 100%. Dental plans may include extra benefits such as LASIK, orthodontia or hearing care.
